Accident Claims

Scope of Cover

Personal Accident Insurance provides following benefits:

Policy sum insured in case of Accidental Death.

  • Permanent Partial Disability (PPD) – as per disability percentage.
  • Permanent Total Disability (PTD) – as per disability percentage.
  • Temporary Total Disability (TTD) – weekly benefits for being on leave due to accident.

Claims Steps

1 :

Upon the happening of an event giving rise to a claim, immediate written Notice should be given to the Insurance Company with all particulars

2 :

For death claims , the intimation should be given within one month

3 :

For Permanent Total Disablement notice to be given within one calendar month.

4 :

For Temporary Total Disablement (TTD) claims, immediate notice should be given to the Insurance Company.

5 :

Any medical or any other representative of the Company will examine the insured person for PTD Claims.

6 :

Proof of income/relationship with assignee shall be furnished.

Documents Required for Death Claim :

1 :

Claim Intimation

2 :

Duly filled and signed claim form

3 :

Original Certificate of Insurance

4 :

Copy of Death Certificate attested by issuing authorities

5 :

Copy of Final Police Report attested by issuing authorities

6 :

Copy of FIR /MLC Copy /Spot Panchnama /Inquest Panchnama attested by issuing authorities Affidavit from the legal heirs of the deceased (in case nomination has not been filed by deceased)

7 :

Copy of Post Mortem Report attested by issuing authorities by issuing authorities

8 :

Legal heir/assignee’s identity proof certificate.
Fitness Certificate.

9 :

Documents related to treatment received attested by issuing authorities/Original

10 :

Please specify the option of claim payment by cheque or NEFT on claim form For NEFT payment, please provide us with copy of cancelled cheque of nominee or copy of front page of bank passbook or letter from bank confirming Account details of nominee with IFSC/ MICR details.

11 :

Investigation reports of treating hospital attested by issuing authorities

12 :

Documents related to treatment received attested by issuing authorities/Original For NEFT payment, please provide us with copy of cancelled cheque and copy of front page of bank passbook.

If you wish to Intimate your claim through SMS, Please SMS “CLAIM” to your insurer. SMS numbers are listed Here.

E-mail

Please Intimate Motor/Health claim via E-Mail. Insurer are now using Quick & Easy eclaim process and can be done round the clock anywhere, anytime at your convenience.
